Abstract

Persistent fever, defined as an elevated body temperature lasting for more than 7–10 days despite appropriate initial evaluation and therapy, represents a significant diagnostic and therapeutic challenge in clinical medicine. When associated with dysregulated immune activation, it often signifies a spectrum of underlying conditions ranging from autoimmune and autoinflammatory diseases to secondary hemophagocytic lymphohistiocytosis (HLH) and macrophage activation syndrome (MAS). This article synthesizes current evidence and guideline-based practices, providing an in-depth, case-based educational review for clinicians. Emphasis is placed on epidemiology, pathophysiology, risk factors, clinical manifestations, diagnostic algorithms, and management strategies, with a focus on recent advances and practical recommendations for navigating these complex presentations.

Introduction

Persistent fever with dysregulated immune activation is increasingly recognized in a variety of clinical contexts, including infectious, neoplastic, and rheumatologic diseases. The diagnostic approach to these patients is inherently complex due to overlapping clinical features and broad differential diagnoses. Recent advancements in immunopathological understanding and biomarker discovery have enhanced our ability to characterize and manage these syndromes. This review leverages a case-based learning framework to illustrate the nuanced clinical reasoning required for optimal patient outcomes.

Epidemiology / Disease Burden

Persistent fever of unknown origin (FUO) accounts for approximately 2–5% of all medical admissions, with a significant proportion attributable to immune dysregulation. In pediatric populations, systemic juvenile idiopathic arthritis (sJIA) and its severe complication, macrophage activation syndrome, are prototypical examples, whereas adults more commonly present with HLH, adult-onset Still’s disease, or undiagnosed systemic autoimmune disorders. The global burden remains underappreciated due to diagnostic challenges, particularly in low-resource settings, resulting in delayed or missed diagnoses and increased morbidity and mortality.

Pathophysiology

Dysregulated immune activation leading to persistent fever often involves a cascade of hypercytokinemia, excessive activation of T lymphocytes and macrophages, and defective cytotoxic function. In HLH and MAS, impaired cytolytic activity of natural killer (NK) cells and cytotoxic T lymphocytes fails to control immune responses, resulting in uncontrolled macrophage proliferation and a cytokine storm. Elevated levels of interleukin (IL)-1, IL-6, IL-18, interferon-gamma, and tumor necrosis factor-alpha drive the febrile response and multi-organ dysfunction. Genetic predispositions, such as mutations in perforin or other cytolytic pathway genes, further contribute to susceptibility, especially in familial cases.

Risk Factors

Recognized risk factors include underlying rheumatologic disorders (e.g., SLE, sJIA), malignancies (notably lymphomas), chronic infections (such as EBV, CMV, TB), immunosuppression, and genetic predispositions. Environmental and iatrogenic factors, such as recent biologic therapy or hematopoietic stem cell transplantation, also enhance risk. A detailed clinical history with attention to disease course, medication exposure, and family history is crucial.

Clinical Features

Patients typically present with high-spiking, remittent or continuous fever, profound fatigue, hepatosplenomegaly, lymphadenopathy, and signs of organ dysfunction. Laboratory findings may reveal cytopenias, elevated ferritin, hypertriglyceridemia, hypofibrinogenemia, and deranged liver function tests. Inflammatory markers such as C-reactive protein and erythrocyte sedimentation rate are markedly elevated. Neurological symptoms, coagulopathy, and acute respiratory distress may indicate advanced disease or multi-organ involvement. The clinical course can be fulminant, underscoring the need for prompt recognition and intervention.

Diagnosis

Diagnosis hinges on a high index of suspicion, especially in the context of suggestive clinical and laboratory findings. Diagnostic criteria such as the HLH-2004 guidelines or MAS classification criteria (e.g., EULAR/ACR/PRINTO) are instrumental. Key investigations include complete blood count with differential, ferritin, triglycerides, fibrinogen, liver enzymes, soluble IL-2 receptor (sCD25), and NK cell activity. Bone marrow aspiration may reveal hemophagocytosis, although its absence does not exclude the diagnosis. Advanced imaging and infection screening are essential to rule out underlying triggers. Molecular and genetic testing is increasingly relevant, particularly in pediatric or familial cases.

Treatment & Management

Management strategies are tailored according to severity, underlying etiology, and organ involvement. Initial therapy often involves high-dose corticosteroids, intravenous immunoglobulin (IVIG), and cyclosporine. In refractory or fulminant cases, etoposide-based regimens (HLH-94/HLH-2004 protocols) are considered, especially in primary or familial HLH. Biologic therapies targeting specific cytokines, such as anakinra (IL-1 inhibitor), tocilizumab (IL-6 inhibitor), and emapalumab (anti-interferon-gamma), have shown efficacy in both MAS and HLH. Supportive care with aggressive management of infections, coagulopathy, and organ dysfunction is critical. Hematopoietic stem cell transplantation remains the definitive therapy for primary, relapsing, or refractory HLH.

Recent Advances / Emerging Therapies

The therapeutic landscape is rapidly evolving with the advent of targeted biologic agents and novel immunomodulators. Janus kinase (JAK) inhibitors and anti-IFN-gamma antibodies are under active investigation and have demonstrated promising results in early-phase trials. Next-generation sequencing is streamlining the identification of genetic mutations and informing personalized therapy. Advances in biomarker discovery, including sCD163 and CXCL9, are enhancing diagnostic precision and prognostication.

Guideline Recommendations

Current guidelines from international bodies such as the Histiocyte Society and EULAR/ACR recommend a staged approach: prompt exclusion of infectious and malignant causes, early initiation of immunosuppressive therapy in suspected MAS/HLH, and referral to specialized centers for refractory cases. Multidisciplinary collaboration, including rheumatology, hematology, infectious diseases, and intensive care, is emphasized. Regular monitoring for adverse effects and therapy response is essential.

Persistent fever with dysregulated immune activation encompasses a spectrum of life-threatening disorders necessitating a high degree of clinical vigilance and multidisciplinary expertise. Recent advances in immunopathology, diagnostics, and therapeutics are improving outcomes, but challenges remain in early recognition and individualized management. Ongoing research and guideline refinement will continue to shape best practices and enhance patient care in this complex area of clinical medicine.
