Parkinson's disease (PD) is a progressive neurodegenerative disorder primarily targeting the dopaminergic neurons in the substantia nigra. Characterized by motor and non-motor symptoms, PD affects approximately 1% of individuals aged 60 and above, presenting significant challenges in diagnosis and treatment.
PD pathogenesis involves the selective loss of dopaminergic neurons in the substantia nigra pars compacta, resulting in dopamine deficiency. The presence of intracellular inclusions called Lewy bodies, primarily composed of alpha-synuclein, is another pathological hallmark. Genetic and environmental factors contribute to PD onset, with several genes identified as significant in familial cases.
Diagnosis of PD is primarily clinical, relying on the identification of cardinal motor symptoms: bradykinesia, resting tremor, and rigidity. Non-motor symptoms such as autonomic dysfunction, cognitive impairment, and mood disorders often precede motor symptoms, further complicating diagnosis. Neuroimaging techniques like DAT-SPECT can support diagnosis, but their use is not routine.
Treatment strategies for PD aim to alleviate symptoms and improve quality of life. Dopaminergic medications, such as levodopa, are the mainstay of treatment. However, long-term use can lead to motor complications including dyskinesias. Non-pharmacological interventions, including physiotherapy and occupational therapy, are crucial adjuncts. Advanced PD may require surgical interventions like deep brain stimulation.
Understanding the complex pathophysiology of PD is key to developing more effective diagnostic tools and therapeutic strategies. Early recognition of non-motor symptoms could lead to earlier diagnosis, potentially slowing disease progression. As our knowledge of PD continues to evolve, a multidisciplinary approach remains central to managing this challenging condition.
