Diagnosis

Accurate and timely diagnosis is essential for effective management of enteric fever. Blood culture remains the gold standard for diagnosis, although its sensitivity can be low. Rapid diagnostic tests (RDTs) are increasingly used due to their ease of use, but their sensitivity and specificity vary. Molecular techniques, such as PCR, offer promise for future diagnostics but require further validation.

Treatment

Antibiotic therapy is the cornerstone of enteric fever treatment. However, the emergence of multidrug-resistant strains necessitates a judicious use of antibiotics. Fluoroquinolones, the third-generation cephalosporins and azithromycin are currently recommended for treatment. It is crucial to monitor the local antimicrobial resistance patterns and adapt the treatment strategy accordingly.

Prevention

Prevention strategies for enteric fever include improved sanitation, safe water supply, and vaccination. Typhoid conjugate vaccines (TCVs), providing longer-term protection and suitable for children under two, have been WHO-prequalified and are recommended for use in endemic areas. However, vaccine coverage remains suboptimal in many regions.

Healthcare Systems Strengthening

Strengthening healthcare systems is key to managing enteric fever effectively. This includes improving laboratory infrastructure for accurate diagnosis, training healthcare workers in case management, and establishing surveillance systems for monitoring disease burden and antimicrobial resistance.
