Antimicrobial resistance (AMR) is a growing global health concern. It threatens the effective prevention and treatment of an ever-increasing range of infections caused by bacteria, parasites, viruses, and fungi. One key strategy to combat this crisis is the implementation of Antimicrobial Stewardship Programs (ASPs) in hospital settings.
ASPs are coordinated interventions designed to improve and measure the appropriate use of antimicrobials by promoting the selection of the optimal antimicrobial drug regimen, dose, duration of therapy, and route of administration. They aim to achieve optimal clinical outcomes related to antimicrobial use, minimize toxicity and other adverse events, reduce the costs of health care for infections, and limit the selection for antimicrobial resistant strains.
Successful ASPs typically involve a multidisciplinary team including infectious disease physicians, pharmacists, clinical microbiologists, and infection preventionists. They leverage hospital informatics systems for surveillance and reporting of antimicrobial usage and resistance patterns, provide regular feedback to prescribers, and offer education and guidelines on optimal antimicrobial use. The use of rapid diagnostic technologies can also enhance the effectiveness of ASPs by enabling more timely and targeted antimicrobial use.
Multiple studies have shown that ASPs can significantly improve patient outcomes. They can reduce the incidence of antimicrobial resistance, Clostridium difficile infection, and other healthcare-associated infections, decrease patient length of stay, and even reduce mortality in some settings. Furthermore, they can lead to substantial cost savings for hospitals.
Given the increasing threat of AMR, the role of antimicrobial stewardship in optimizing patient care in hospital settings is more crucial than ever. It requires a concerted, multidisciplinary effort, but the potential benefits in terms of improved patient outcomes and cost savings are substantial. As healthcare professionals, we must advocate for and support the implementation and ongoing development of ASPs in our institutions.
