Thyroid nodules, discrete lesions within the thyroid gland, are common clinical findings. While most are benign, a small proportion may harbor malignancy, necessitating a thorough evaluation. This article offers a comprehensive guide to assessing and interpreting thyroid nodules.
Thyroid nodules are prevalent in 4-7% of adults based on palpation, but the prevalence increases to 19-67% when using ultrasound. Most nodules are asymptomatic and discovered incidentally. However, some may present with compressive symptoms, thyroid dysfunction, or be associated with malignancy.
The first step in evaluation is a detailed history and physical examination. High-resolution thyroid ultrasound is the imaging modality of choice. It helps determine the size, number, echogenicity, and other characteristics of the nodule, which are crucial in risk stratification.
Fine Needle Aspiration (FNA) is a key procedure in the evaluation of thyroid nodules. It is typically recommended for nodules >1cm with suspicious ultrasound features or >1.5cm without such features. The cytologic result guides further management.
The Bethesda System for Reporting Thyroid Cytopathology is widely used. It categorizes results into six groups, each associated with a specific malignancy risk and recommended management. However, interpretation should be individualized considering the patient's clinical context.
Management options range from surveillance to surgery, depending on the cytologic result, nodule size, ultrasound features, and patient's preference. In some cases, molecular testing may be useful to further stratify the risk of malignancy.
Assessing and interpreting thyroid nodules requires a systematic approach, integrating clinical, ultrasonographic, and cytologic data. Understanding this process is essential for healthcare professionals to ensure appropriate management and follow-up of patients with thyroid nodules.
