Cardiometabolic disorders during pregnancy, including gestational diabetes mellitus (GDM), hypertensive disorders, and dyslipidemia, pose significant risks to maternal and fetal health. This review synthesizes the latest guidelines and evidence regarding optimal cardiometabolic monitoring strategies in pregnant women, emphasizing the importance of early identification, risk stratification, and evidence-based management. A clear understanding of epidemiological trends, pathophysiology, and clinical features guides the diagnostic process and informs therapeutic decisions. Emerging research and updated recommendations are discussed to support clinicians in providing high-quality, guideline-adherent care during pregnancy.
Cardiometabolic complications, notably hypertensive disorders of pregnancy, gestational diabetes, and abnormal lipid profiles, have become increasingly prevalent in obstetric care. The physiological changes of pregnancy can unmask or exacerbate underlying metabolic vulnerabilities, contributing to both short- and long-term morbidity for mother and child. Recent clinical guidelines underscore the necessity of systematic monitoring to identify at-risk individuals, allowing for timely interventions. This review aims to provide healthcare professionals with a comprehensive, evidence-based overview of current recommendations for cardiometabolic monitoring during pregnancy, integrating recent advances and highlighting practical clinical implications.
Globally, an estimated 10–15% of pregnancies are complicated by cardiometabolic disorders, with gestational diabetes mellitus affecting up to 14% and hypertensive disorders impacting 5–10% of pregnancies. The increasing prevalence is partly attributable to rising maternal age, obesity rates, and sedentary lifestyles. These conditions elevate the risk for adverse maternal outcomes such as preeclampsia, eclampsia, and future cardiovascular disease, while also contributing to fetal risks including macrosomia, preterm birth, and long-term metabolic sequelae. Epidemiological data highlight the disproportionate burden in certain populations, including ethnic minorities and individuals with limited access to prenatal care, underscoring the importance of targeted monitoring and intervention strategies.
Pregnancy induces profound physiological adaptations, including increased insulin resistance, augmented blood volume, and altered lipid metabolism. In susceptible individuals, these changes precipitate or worsen metabolic derangements. Insulin resistance is driven by placental hormones such as human placental lactogen, while endothelial dysfunction and altered vascular reactivity contribute to hypertensive disorders. Dyslipidemia during pregnancy, particularly elevated triglycerides, reflects both hormonal influences and preexisting metabolic risk. Understanding these pathophysiological mechanisms informs risk stratification and the selection of appropriate monitoring tools.
Major risk factors for cardiometabolic complications during pregnancy include advanced maternal age, pre-pregnancy obesity (BMI ≥30 kg/m²), a history of GDM or preeclampsia, family history of diabetes or cardiovascular disease, polycystic ovary syndrome, and certain ethnic backgrounds (e.g., South Asian, African American, Hispanic). Lifestyle factors such as poor diet, physical inactivity, and smoking further compound risk. Early identification of these risk factors is essential for individualized monitoring protocols and timely intervention.
Clinical manifestations of cardiometabolic disorders during pregnancy are often subtle and may overlap with normal gestational changes. GDM typically presents without overt symptoms but may be associated with excessive fetal growth or polyhydramnios. Hypertensive disorders manifest as elevated blood pressure, proteinuria, and, in severe cases, symptoms such as headache, visual disturbances, and edema. Lipid abnormalities are usually asymptomatic but may contribute to other complications if severe. Vigilant clinical assessment and awareness of risk profiles are crucial for early detection.
Guidelines recommend universal screening for GDM between 24–28 weeks gestation using either a one-step (75g oral glucose tolerance test) or two-step approach (50g glucose challenge test followed by 100g OGTT if positive). Hypertensive disorders are diagnosed based on serial blood pressure measurements (≥140/90 mmHg) and assessment for proteinuria and organ dysfunction. Lipid profiles are not routinely measured unless there is a clinical indication, but baseline assessment may be warranted in high-risk patients. Early and regular monitoring, combined with appropriate laboratory testing, is fundamental for timely diagnosis and management.
Management strategies for cardiometabolic disorders in pregnancy are guided by disease severity and gestational age. GDM is managed with dietary modification, physical activity, glucose self-monitoring, and, when necessary, pharmacotherapy such as insulin or select oral agents. Hypertensive disorders require blood pressure control with pregnancy-safe antihypertensives (e.g., labetalol, nifedipine), close fetal surveillance, and timely delivery in severe cases. Lifestyle interventions are cornerstone for all patients, with multidisciplinary care involving obstetricians, endocrinologists, and dietitians. Postpartum follow-up is critical given the long-term risk for type 2 diabetes and cardiovascular disease.
Recent research has focused on refining risk stratification tools, integrating biomarkers such as placental growth factor for preeclampsia prediction, and novel continuous glucose monitoring systems for GDM management. There is growing interest in individualized lifestyle interventions and digital health platforms to enhance patient engagement. Pharmacological advances include the exploration of newer antihypertensives and glucose-lowering agents with improved safety profiles for pregnancy. Emerging data also support the role of preconception counseling and optimization of metabolic health prior to pregnancy to reduce perinatal risks.
Leading organizations, including the American College of Obstetricians and Gynecologists (ACOG), the International Association of Diabetes and Pregnancy Study Groups (IADPSG), and the International Society for the Study of Hypertension in Pregnancy (ISSHP), recommend risk-based assessment at the first prenatal visit, universal GDM screening at 24–28 weeks, and regular blood pressure monitoring throughout gestation. Individualized monitoring protocols are advised for high-risk patients, with emphasis on timely intervention and multidisciplinary management. Postpartum follow-up for cardiometabolic risk reduction is universally endorsed.
Cardiometabolic monitoring during pregnancy is essential for optimizing maternal and fetal outcomes. Adherence to evidence-based guidelines enables early identification and management of at-risk women, mitigating both immediate and future health risks. Ongoing research and emerging technologies promise to further refine monitoring strategies, supporting the delivery of personalized, high-quality care in obstetric practice.
