Diagnosis of hypertensive emergencies requires careful clinical evaluation and appropriate investigations. A thorough history and physical examination are paramount. Laboratory investigations, including serum creatinine, electrolytes, complete blood count, and urinalysis, along with imaging studies such as chest X-ray or CT scan, can provide valuable information about the extent of organ damage.
Management of hypertensive emergencies involves immediate reduction of blood pressure, typically by 25% in the first hour, followed by gradual reduction over the next 24-48 hours. Intravenous antihypertensive agents are preferred. The choice of agent depends on the clinical scenario, with options including sodium nitroprusside, labetalol, and nicardipine. It is crucial to avoid overly rapid reduction in blood pressure, which can lead to ischemic complications.
Prevention of hypertensive emergencies involves optimal control of blood pressure in patients with known hypertension. Regular follow-up, lifestyle modifications, and adherence to antihypertensive medications are key. Patients who have experienced a hypertensive emergency require close monitoring and management of potential long-term complications.
