Abstract

Polycystic Ovary Syndrome (PCOS) is a prevalent and complex endocrine disorder with significant reproductive, metabolic, and psychological implications. Its heterogeneity demands a multidisciplinary strategy encompassing accurate diagnosis, individualized management, and long-term monitoring. Recent advances have elucidated the intricate pathophysiology and expanded therapeutic options, yet clinical challenges persist in optimizing patient outcomes. This review synthesizes contemporary evidence, guideline-based recommendations, and practical insights for clinicians managing PCOS, emphasizing the importance of integrated care and ongoing research.

Introduction

PCOS is recognized as one of the most common endocrinopathies among women of reproductive age, with multifaceted clinical manifestations affecting diverse organ systems. Beyond reproductive dysfunction, the syndrome is closely linked to metabolic derangements, cardiovascular risk, and psychosocial morbidity. Recent years have witnessed significant progress in understanding its underlying mechanisms and therapeutic approaches. This review presents an up-to-date, multidisciplinary perspective on PCOS, with a focus on evidence-based diagnosis, long-term management, and emerging strategies for improving patient care.

Epidemiology / Disease Burden

The global prevalence of PCOS is estimated to range from 6% to 21%, depending on diagnostic criteria (NIH, Rotterdam, or AES). It represents a leading cause of infertility and a major contributor to metabolic syndrome in women. The syndrome imposes substantial economic and psychosocial burdens, with higher healthcare utilization and diminished quality of life. Pathophysiology

Psychological comorbidities, including anxiety, depression, and diminished health-related quality of life, are frequently encountered and necessitate comprehensive care.

Diagnosis

Diagnosis is based on a combination of clinical, biochemical, and imaging criteria. The 2003 Rotterdam consensus (requiring two of three: oligo/anovulation, clinical/biochemical hyperandrogenism, polycystic ovaries) remains widely adopted, although debates persist regarding phenotypic heterogeneity. Exclusion of secondary etiologies (thyroid dysfunction, hyperprolactinemia, non-classical congenital adrenal hyperplasia, androgen-secreting tumors) is essential. Biochemical evaluation should include total and free testosterone, LH, FSH, prolactin, TSH, and 17-hydroxyprogesterone. Transvaginal ultrasonography aids in assessing ovarian morphology, but interpretation must consider age and menstrual status. Emerging biomarkers and imaging modalities hold promise for refining diagnostic accuracy.

Treatment & Management

Management of PCOS is tailored to the patient\'s phenotype, symptomatology, and reproductive goals. Lifestyle modification remains foundational, particularly for overweight or obese women, with evidence supporting even modest weight loss in ameliorating metabolic and reproductive outcomes. First-line pharmacotherapy for menstrual irregularity and hyperandrogenism includes combined oral contraceptives, which regulate cycles and suppress androgen excess. Anti-androgens (spironolactone, finasteride) may be considered for refractory hirsutism, with appropriate contraception. For women seeking fertility, ovulation induction with letrozole is preferred over clomiphene citrate, supported by superior live birth rates. Metformin is beneficial for metabolic dysfunction and may be adjunctive in select cases. Bariatric surgery is reserved for severe obesity with comorbidities. A multidisciplinary approach—including endocrinology, gynecology, dermatology, reproductive medicine, and mental health support—is paramount for optimal long-term care.

Recent Advances / Emerging Therapies

Recent years have seen advances in understanding PCOS pathophysiology and therapeutic innovation. Inositol isomers (myo- and D-chiro-inositol) have shown promise in improving ovulatory function and insulin sensitivity. Glucagon-like peptide-1 (GLP-1) receptor agonists, initially developed for diabetes, demonstrate beneficial effects on weight reduction and metabolic parameters in PCOS. Research into selective androgen receptor modulators, anti-inflammatory agents, and gut microbiota modulation continues. Novel imaging and omics-based diagnostics may further individualize care in the future. Despite these advances, robust long-term outcome data are needed to inform clinical practice.

Guideline Recommendations

International guidelines (e.g., ESHRE/ASRM, Endocrine Society) advocate a holistic, patient-centered approach, emphasizing early diagnosis, lifestyle intervention, and multidisciplinary care. Regular screening for metabolic and cardiovascular risk factors is recommended. Individualization of treatment, shared decision-making, and attention to mental health are essential. Periodic reassessment and adaptation of management strategies ensure responsiveness to evolving patient needs across the lifespan.

PCOS is a prevalent, multifaceted disorder necessitating a comprehensive, multidisciplinary approach for effective diagnosis and long-term management. Advancements in understanding its pathophysiology and expanding therapeutic options enhance patient care, but ongoing research and individualized strategies remain critical. Early recognition, integrated management, and patient education are key to improving clinical outcomes and quality of life for women with PCOS.
