Fine-needle aspiration (FNA) is the most reliable method for differentiating benign from malignant nodules. The Bethesda System for Reporting Thyroid Cytopathology is commonly used to categorize FNA results, guiding subsequent management decisions.
Advances in genomics have led to the development of molecular tests that can further refine the risk of malignancy in indeterminate nodules. These tests analyze the nodule's genetic material for mutations or rearrangements associated with thyroid cancer.
The management of thyroid nodules is tailored to the individual patient, taking into account the clinical, radiological, and cytological findings. Options range from watchful waiting to surgery, with thyroid hormone suppression therapy and radioiodine treatment as intermediate steps.
Thyroid nodule evaluation is a complex, multidisciplinary process that requires a comprehensive understanding of clinical, radiological, and pathological assessments. With the advent of molecular testing, the diagnostic accuracy has improved, enabling more personalized management strategies and ultimately improving patient outcomes.
