A thorough clinical history and physical examination is the cornerstone of evaluating abdominal pain. The onset, location, character, and radiation of the pain, along with associated symptoms, can provide valuable clues towards the underlying cause. Physical examination should include inspection, palpation, percussion, and auscultation, with special attention to signs of acute abdomen.
Basic laboratory tests, including complete blood count, liver function tests, renal function tests, and urinalysis, can aid in diagnosis. Imaging studies such as ultrasound, computed tomography (CT), and magnetic resonance imaging (MRI) can further delineate the cause of pain. Endoscopic procedures may be warranted in certain cases.
Special considerations should be given to certain populations. In women of childbearing age, gynecological causes should be considered. In elderly patients, vascular causes like mesenteric ischemia should be in the differential diagnosis. In patients with a history of surgery, postoperative complications or adhesions could be the cause.
Management should be tailored according to the underlying cause. This may range from conservative management with analgesics and dietary modifications, to surgical intervention in cases of acute abdomen. Referral to a specialist may be necessary in complex cases.
The evaluation of abdominal pain requires a systematic approach, beginning with a thorough clinical history and examination, followed by appropriate investigations.
