Abstract

Chronic kidney disease (CKD) represents a significant and growing public health concern worldwide, disproportionately affecting marginalized and underserved communities. This review explores evidence-based, equitable community strategies aimed at preserving renal health, emphasizing the integration of population-level interventions, social determinants of health, and guideline-driven clinical practices. Drawing upon recent epidemiological data and mechanistic insights, this article discusses risk stratification, early detection, and multidisciplinary management, with a focus on translating emerging research and public health frameworks into actionable, locally adapted programs. The overarching goal is to inform healthcare professionals about the clinical and public health implications of targeted, equity-oriented strategies to reduce the burden of renal disease and improve community outcomes.

Introduction

Renal health is foundational to overall well-being, yet chronic kidney disease remains under-recognized and under-addressed in many populations, particularly among those experiencing socioeconomic disadvantage. Public health approaches that prioritize equity are crucial for mitigating the rising incidence and adverse outcomes associated with CKD. This article synthesizes contemporary research and guidelines to provide clinicians and public health professionals with a strategic framework for community-based renal health preservation, highlighting the intersection of clinical care, social determinants, and health systems strengthening.

Epidemiology / Disease Burden

CKD affects approximately 10-15% of the global adult population, with prevalence rates rising due to aging demographics, increasing rates of diabetes and hypertension, and persistent gaps in healthcare access. Epidemiological studies reveal marked disparities in CKD incidence, progression, and outcomes, with minority and low-income groups experiencing higher disease burden. In the United States, African Americans, Hispanic Americans, and Native Americans are disproportionately affected, attributed to a complex interplay of genetic, socioeconomic, and environmental factors. Community-based screening programs, especially in primary care and outreach settings, are effective for early identification and risk assessment, facilitating timely intervention.

Treatment & Management

Management of CKD encompasses risk factor modification, pharmacologic intervention, and multidisciplinary care. Blood pressure control, glycemic optimization in diabetics, and use of RAAS inhibitors remain foundational. Community strategies include task-shifting to non-physician health workers, mobile clinics, and telemedicine to expand access. Nutritional counseling, smoking cessation, and patient education should be culturally adapted and address local barriers. Coordination with social services and community organizations enhances continuity of care and addresses social determinants impacting adherence and outcomes.

Recent Advances / Emerging Therapies

Recent advances in CKD management include the introduction of sodium-glucose cotransporter 2 (SGLT2) inhibitors, which demonstrate renoprotective effects independent of glycemic control, and non-steroidal mineralocorticoid receptor antagonists. Novel biomarkers, such as soluble urokinase plasminogen activator receptor (suPAR) and proteomic profiling, are under investigation for earlier detection and risk stratification. Digital health platforms and artificial intelligence-driven predictive models enhance population screening and personalized intervention strategies. Community-based participatory research is emerging as a powerful approach to co-design interventions with affected populations, improving engagement and sustainability.

Guideline Recommendations

International guidelines from KDIGO, the American Diabetes Association (ADA), and the World Health Organization (WHO) underscore the importance of early identification, equitable access to care, and integration of community resources. Recommended strategies include annual screening for at-risk individuals, aggressive management of hypertension and diabetes, and multidisciplinary care coordination. Tailoring guidelines to local contexts, addressing barriers such as cost, transportation, and health literacy, is essential for equitable implementation. Policy advocacy for universal health coverage and investment in primary care infrastructure are critical public health imperatives.
