Thyroid nodules, a common clinical problem, often pose diagnostic challenges. Comprehensive evaluation is vital to differentiate benign nodules from those requiring surgical intervention. This guide offers a structured approach to thyroid nodule evaluation.
Thyroid nodules are prevalent in up to 68% of the population, with a higher incidence in women, the elderly, and individuals with iodine deficiency. Risk factors include radiation exposure, family history of thyroid cancer, and certain genetic syndromes.
Clinical evaluation involves a thorough history and physical examination. Symptoms such as dysphagia, voice changes, and rapid nodule growth may suggest malignancy. Physical examination should assess nodule size, consistency, mobility, and presence of cervical lymphadenopathy.
Ultrasonography is the first-line imaging modality for thyroid nodules, providing information about nodule size, composition, echogenicity, and vascularity. Suspicious sonographic features include microcalcifications, irregular margins, taller-than-wide shape, and hypoechoic nature.
Fine-needle aspiration (FNA) biopsy is the most reliable method for distinguishing benign from malignant nodules. Cytological evaluation classifies nodules into six categories, guiding management decisions.
Management depends on FNA cytology, sonographic features, and clinical factors. Options include surveillance, thyroid hormone suppression therapy, ethanol ablation, radioiodine therapy, or surgery.
Thyroid nodule evaluation requires a systematic approach, integrating clinical, sonographic, and cytological findings to guide management. Ongoing research and technological advancements promise to further refine this process, improving patient outcomes.
