Early recognition and management of stroke are critical to reducing morbidity and mortality. This piece aims to provide healthcare professionals with a detailed understanding of the early signs of stroke and the necessary steps for prompt management.
Stroke symptoms typically occur suddenly. The acronym FAST (Face drooping, Arm weakness, Speech difficulties, Time to call emergency services) is a useful tool for early recognition. However, other symptoms such as sudden confusion, trouble seeing, difficulty walking, dizziness, and severe headache with no known cause can also indicate a stroke.
Upon suspecting a stroke, a rapid neurological assessment should be performed. This includes the National Institutes of Health Stroke Scale (NIHSS). Imaging studies, such as a non-contrast CT scan, should be ordered immediately to differentiate between ischemic and hemorrhagic stroke. Blood tests, ECG, and carotid ultrasound may also be useful.
Time is of the essence in stroke management. In ischemic stroke, thrombolytic therapy should be initiated within 4.5 hours of symptom onset. In hemorrhagic stroke, controlling blood pressure and reversing anticoagulation are crucial. All stroke patients should receive oxygen if hypoxic, and glucose control should be optimized.
After initial management, attention should be turned to secondary prevention. This includes the use of antiplatelet agents in ischemic stroke, control of hypertension, statin therapy, and lifestyle modifications such as smoking cessation, healthy diet, and regular exercise.
Early recognition and prompt management of stroke can significantly improve patient outcomes. It is imperative for healthcare professionals to be well-versed in the signs, assessment, and treatment of stroke, as well as strategies for secondary prevention.
