Access to assisted reproductive technologies (ART) remains unevenly distributed globally, with significant disparities persisting along socioeconomic, racial, and geographic lines. Public health programs are increasingly being tasked with the mandate to bridge these gaps and promote reproductive health equity. This review examines the epidemiology of infertility, underlying mechanisms, risk factors, and the clinical and public health burden associated with reproductive inequities. We explore how targeted public health initiatives and emerging ART advancements are shaping more inclusive access and improving outcomes, guided by the latest clinical practice recommendations and evidence-based strategies.
Infertility affects millions worldwide, posing not just a medical challenge but also a profound psychosocial and public health issue. Disparities in access to ART including in vitro fertilization (IVF), intrauterine insemination (IUI), and advanced gamete preservation are driven by multifactorial determinants such as income, education, race, and healthcare system limitations. Public health programs have the potential to reduce these disparities by implementing evidence-based policies, facilitating equitable access, and supporting clinical innovation. This article synthesizes current knowledge and recent developments in the field, emphasizing a multidisciplinary approach towards reproductive health equity.
Infertility affects approximately 8-12% of reproductive-aged couples worldwide, with higher prevalence in low- and middle-income countries (LMICs) due to untreated infections, limited access to healthcare, and sociocultural barriers. According to the World Health Organization, over 48 million couples and 186 million individuals live with infertility globally. The burden is amplified among historically marginalized populations, including racial and ethnic minorities. Disparities persist in ART utilization: for instance, Black and Hispanic women in the United States are less likely to receive infertility treatments and experience lower success rates compared to their White counterparts. The cumulative impact results in significant psychological distress, economic hardship, and reduced quality of life, underlining the urgent need for health equity interventions.
The pathophysiology of infertility encompasses diverse etiologies including ovulatory dysfunction, tubal factor infertility, diminished ovarian reserve, male factor infertility, and unexplained causes. Socioeconomic and environmental factors such as limited access to timely diagnosis, untreated reproductive tract infections, and exposure to environmental toxins exacerbate biological vulnerabilities, especially in disadvantaged communities. Mechanistically, chronic stress, malnutrition, and inadequate healthcare exacerbate hormonal imbalances and reproductive system damage, further impeding fertility in underserved populations.
Key risk factors for infertility include advanced maternal age, obesity, sexually transmitted infections (STIs), polycystic ovary syndrome (PCOS), endometriosis, lifestyle factors (smoking, excessive alcohol use), and environmental exposures. Social determinants such as poverty, lack of health insurance, and geographic remoteness compound biological risks by restricting access to preventive care and timely ART interventions. Racial and ethnic minorities often face structural barriers including discrimination, language differences, and lower health literacy, contributing to delayed or suboptimal infertility care.
Patients with infertility typically present with an inability to achieve pregnancy after 12 months of regular unprotected intercourse, or after six months in women over age 35. Clinical features may include menstrual irregularities, pelvic pain, or a history of STIs, depending on the underlying etiology. In many cases, affected individuals experience psychological symptoms such as anxiety, depression, and stigma, particularly when access to ART is limited or denied. Recognizing these multifaceted clinical features is critical for comprehensive patient assessment and management.
Infertility evaluation involves a systematic assessment of both partners. Standard diagnostic protocols include semen analysis, assessment of ovulatory function, tubal patency testing (e.g., hysterosalpingography), and ovarian reserve testing (AMH, FSH levels, antral follicle count). In resource-limited settings, diagnostic capabilities may be constrained, necessitating public health interventions to improve laboratory infrastructure and provider training. Timely and equitable diagnosis is essential for optimizing ART outcomes and reducing disparities.
Management strategies are tailored to etiology, patient age, and individual preferences. ART options include ovulation induction, IUI, IVF, and third-party reproduction (donor eggs/sperm, gestational surrogacy). Cost remains a major barrier, as ART is often not covered by insurance in many regions. Public health programs have increasingly adopted subsidization schemes, sliding-scale payment models, and integration of infertility care into primary health services to enhance accessibility. Community outreach and culturally sensitive counseling further support equity in treatment uptake and adherence.
Recent innovations in ART include pre-implantation genetic testing, time-lapse embryo imaging, and the use of artificial intelligence algorithms to optimize embryo selection. Techniques such as in vitro maturation, oocyte vitrification, and microfluidic sperm sorting are expanding options for patients with complex infertility. Mobile health (mHealth) platforms and telemedicine are being leveraged to deliver reproductive health education and facilitate remote ART consultations, addressing geographic and resource-based barriers. Public health programs are increasingly collaborating with academic centers to pilot and scale these advances in diverse populations.
Professional societies such as the American Society for Reproductive Medicine (ASRM) and the European Society of Human Reproduction and Embryology (ESHRE) emphasize early identification of at-risk populations, equitable access to evidence-based diagnostics and therapies, and culturally competent care. Guidelines recommend integrating infertility services into broader reproductive and sexual health programs, advocating for insurance coverage, and supporting research on disparities in ART outcomes. Public health agencies are urged to collect and report disaggregated data to monitor progress and inform policy interventions.
Advancing reproductive health equity through public health programs and assisted technologies requires sustained commitment to evidence-based policy, clinical innovation, and culturally informed care. By addressing the multifactorial barriers to ART access and harnessing emerging technologies, public health initiatives can substantially reduce disparities and improve reproductive outcomes for all populations. Ongoing research, provider education, and community engagement will be critical to achieving lasting reproductive health equity.
