The increase in cancer survival rates, owing to advancements in early detection and effective therapies, has shifted attention toward the long-term health and quality of life of cancer survivors. Survivorship care includes monitoring for recurrence, managing late effects of treatment, addressing psychosocial concerns, and promoting health maintenance. The complexity of survivorship care necessitates a structured, evidence-based approach tailored to individual patient needs, ensuring seamless integration into clinical practice for optimal patient outcomes.
Globally, the population of cancer survivors is rapidly expanding, with estimates suggesting over 20 million survivors in the United States alone by 2026. This rise is attributed to improved cancer detection, enhanced therapeutic options, and an aging population. Survivors face a unique disease burden characterized by an increased risk of secondary malignancies, chronic comorbidities, and treatment-related adverse effects. The societal and healthcare impact is considerable, necessitating dedicated resources, specialized programs, and policy-level interventions to address the growing needs of this demographic.
Cancer therapies—including surgery, radiotherapy, chemotherapy, and targeted agents—exert both acute and long-term effects on normal tissues. Mechanistically, these interventions can induce cellular senescence, DNA damage, immune dysregulation, and chronic inflammation, predisposing survivors to organ dysfunction, metabolic syndrome, and cardiovascular disease. Additionally, immunotherapies and targeted agents continue to evolve, necessitating surveillance for novel long-term toxicities. Ongoing research into interventions such as neurocognitive rehabilitation and integrative therapies holds promise for improving survivor outcomes.
Professional bodies such as the American Society of Clinical Oncology (ASCO) and the National Comprehensive Cancer Network (NCCN) have issued evidence-based guidelines emphasizing risk-based, patient-centered survivorship care. Recommendations include stratified surveillance schedules, cardiovascular and metabolic risk assessment, psychosocial screening, cancer screening (secondary malignancies), and health promotion interventions. Implementation requires systematic integration of survivorship care into routine oncology and primary care workflows, along with education of healthcare providers and patients regarding survivorship issues.
Survivorship-focused cancer care represents a critical component of comprehensive oncology practice. With the increasing population of cancer survivors, clinicians must adopt a structured, guideline-driven approach to address the long-term physical, psychological, and social needs of this cohort.
